How to Submit Your Plan

What to submit for your plan review:

As a healthcare facility licensed by the Agency for Healthcare Administration (AHCA), operating within the State of Florida * and Broward County*, you are required to have and maintain an up-to-date Comprehensive Emergency Management Plan (CEMP) for your facility.

The Agency for Healthcare Administration (AHCA) provides the comprehensive emergency management planning criteria. You are required to follow the AHCA criteria for your facility type. Please ensure you have answered all sections of the criteria.

AHCA CEMP Criteria:

The following links will provide you the current AHCA criteria to be used in the preparation of your facility CEMP plan. Please select your facility type to be linked to the appropriate criteria.

  The following items are required when submitting your plan for review:
  

  1. Your completed AHCA criteria; indicate the section and page number in your plan where the specific information can be located.
  2. A copy of the Broward County Checklist*. Please ensure you include all items required on the checklist.
  3. Your CEMP plan.
  4. All supporting documentation as listed on the last page of the AHCA criteria, under Annexes.

  Submit Your Plan *

  Mail your plan! *

  Do not mail in your plan. Please contact Broward County Emergency Management Division.

  Beginning in April 2023, we are transitioning to the new Broward County electronic Healthcare Facility CEMP Portal. Broward County Emergency Management Division no longer accept plans for review in hardcopy. Further information on the Healthcare CEMP Portal can be found by selecting the CEMP Portal tile below.

  • Do not mail your plan.
  • Do not drop off your plan in person.
  • Do not fax or e-mail your plan.
  • You will receive a notification by e-mail to the address we have on file for your facility administrator, when your plan has been checked into our system.
  • The person who is assigned to review your plan will reach out to you directly via phone or e-mail, during the review process.
  • Allow up to 60 days for your plan to be reviewed.

  Beginning July 1st, 2023, all annual CEMP are required to be submitted to the County through the new electronic CEMP Portal. Facilities will be invited to transition into the CEMP Portal by the County CEMP Healthcare Coordinator via email (EOC-HealthCEMP@broward.org) monthly.

  New facilities in Broward County are required to submit their plans, Comprehensive Emergency management Plan (CEMP) and/or Emergency Environmental Control Plan (EECP).

  Contact Broward County, EOC-HealthCEMP@broward.org, and request additional information. Provide Facility Administrator/Owner, proposed name of facility, type of facility, phone number, and email address. The county will contact and provide additional information to register for the electronic CEMP Portal.​
